These conditions can be given to you on request in paper format. Any act performed on a patient as part of the care activity is subject to these general operating conditions.

 

  1. Definition of care activity

     

The care activity concerned by these general operating conditions is that of Dr Magali MARY, home veterinarian, whose head office is located at 19 chemin d'Ayalou 34620 Puisserguier. According to article R. 242-57 of the Veterinary Code of Ethics, "Is called a home veterinarian the natural or legal person authorized to practice medicine and surgery on animals who, not practicing in a veterinary care establishment , exercises his profession at the Customer's home. »

   2. Usual opening hours and reception conditions for the public

 

The telephone reception and consultations are open Monday to Friday from 9 a.m. to 7 p.m., consultations take place only at the Customer's home.

 

 

   3. Animal Care Personnel

 

Veterinary Dr Magali MARY, graduated from the Faculty of Veterinary Medicine of Liège in 2012, order number 25534.

Training in the DIE of Veterinary Osteopathy at the IMAOV from 2016 to 2018.

Training in Veterinary Acupuncture at ENVT in April 2021.

 

 

   4. Services provided

  •   General and preventive medicine consultations.

  •   Analysis laboratory:

o At the patient's bedside: urine analyses, biochemical analyses: blood sugar (Alphatrak 2), blood smears, skin or ear samples, coproscopy, rapid tests.

o Deferred with results during the day: blood biochemistry (KBIO 5), formula count (KHEMA 4) and endocrinology (KIMMUNO 1) examinations, microscopic examination of a blood smear, skin or ear sample.

o The other analyzes (cytology, histology, bacteriology/parasitology/mycology, treatment controls) will be carried out in third-party laboratories, with the Customer's agreement.

  •   Imaging: bedside ultrasounds (DP-50 vet), other imaging examinations (x-ray, scanner) will be performed by a third-party healthcare facility with the Client's agreement.

  •   Blood pressure measurement (petMAP G3).

  • Tranquillization: when the patient's condition requires and allows it, sedation may be implemented. The protocol will be chosen according to the nature of the intervention, and the state of health of the animal.

  • Consultations in osteopathy and acupuncture.

  • Delivery of medicines, hygiene products and food, in accordance with the legislation on veterinary pharmacy in force.

    Heavier or specialized surgical procedures and hospitalizations will be carried out in third-party healthcare establishments, with the Client's agreement.

 

 

   5. Permanence and continuity of care

 

In order to ensure, in my absence, optimum monitoring of the animals entrusted to me, I have signed an emergency management contract with the following structure: Urgences Vet': located at 8, rue Roque Segui 34420 Villeneuve les Béziers 04 67 01 29 22, clinic dedicated only to emergencies open nights from 7 p.m. until 8 a.m. the next day, weekends from Saturday 2 p.m. to Monday morning 8 a.m. and public holidays from 7 p.m. the day before and until 8 a.m. the next day or immediately following Monday at 8 a.m.

The emergency service takes over my care activity during closing hours. His contact is always available on my answering machine.

 

 

   6. Species treated

 

    The species usually or occasionally processed in my establishment are as follows:

    -  Dogs,

      -  Cats,

      -  Ferrets,

      -  Rodents,

      -  Lagomorphs,

      -  Ornamental birds, hens

      -  Turtles.

        I do not have the equipment and skills necessary to provide care for species not mentioned above, in particular equines and livestock. In the event of an emergency concerning these species, you can contact Dr Vétérinaire Matthieu Daure, 7, Impasse des jardins 34500 Béziers on 04 67 62 34 16.

         

         

           7. Therapeutic risk, anesthetic risk, risk related to restraint, informed consent

         

          Any drug treatment, any anesthesia, any surgical procedure involves a potential therapeutic risk of which I will inform the Client. This information will be provided verbally or in writing in the form of a care contract and/or a detailed estimate (see chapter care contract).

          The aggressive behavior of a patient sometimes requires the use of means of restraint which can lead to injury to the latter and/or the nursing staff. In this case, I will inform the Customer of the need to use a particular restraint for security reasons. The examination of the patient will only be carried out in the event of acceptance of the restraint by the Client.

          The Client declares to have read and accepted the therapeutic risks and, where applicable, the special conditions for examination under restraint set out above.

           

           

             8. Care contract, special conditions

           

          As soon as an animal is entrusted to me, a tacit or written care contract is established implying for the service provider the implementation of the means necessary to carry out the care, and for the owner the payment of the fees at the time of the recovery of the animal. A written estimate can be drawn up on simple request from the owner. This quote is an estimate and may include overruns. In the event of overruns of more than 20% of the amount initially planned, the prior agreement of the owner must be obtained before carrying out the additional services. Any medical or surgical intervention which will be the subject of special conditions not specified or not mentioned in this document will give rise to the establishment of a care contract and/or a detailed estimate. 

          The latter will provide the Client with the information necessary to obtain his informed consent.

           

             

               9. Death of the animal

            ​​

            In the event of the death of the patient, I can, if the legislation allows it and if the Client wishes, return the body for burial purposes. In other cases, I can ensure through the Esthima Company in Nîmes (Actiparc le Grézan, rue Nicolas Appert 30000 Nîmes 04 11 94 02 19) the plural, individual or private cremation of the body. Any request for incineration must be in writing and signed by the Customer.

            The incineration costs are the responsibility of the Customer.

             

             

               10. Tariff conditions

             

              The prices for the main acts can be consulted at the following address: www.ostehomevet.fr. All the prices are available to the Customer in the form of a price list available on request. The total to be paid corresponds to the sum of the medical and surgical services and of the drugs and other products delivered. It will give rise to the production of a detailed invoice in accordance with the legislation. The random nature of certain medical or surgical acts makes it difficult to list them or even their exact costing. In this case, an estimate including financial levels will be given to the Customer, each overrun of the levels must give rise to a new acceptance by the Customer.

               

               

                 11. Terms of payment

               

                The fees are payable in cash at the end of the consultation or when the animal is returned to the Client. Payments by credit card, checks, cash or bank transfer are accepted.

                Exceptionally, deferred payment may be accepted according to the terms agreed between the Veterinarian and the Client.

                A deposit may be requested in particular when several successive visits are planned or as part of a long-term care protocol.

                A credit can be established in the event of refunds of food not started and purchased for less than a month. Medications that have not been started can be taken back under certain conditions to be discussed with the veterinarian. 

                The veterinarian reserves the right to sue the debtor in the event of non-payment.  

                 

                 

                   12. Privacy Policy

                 

                  I have a computer system intended to facilitate the management of patient files and to ensure the invoicing of acts, veterinary medicines and other products.

                  As part of optimized patient monitoring, some of this data may be transmitted to other veterinarians or veterinary laboratories, with the Client's agreement.

                  During the identification of a patient by radiofrequency, certain data is transferred to the national identification file, as provided for by articles D212-63, D212-66 and D212-68 of the Rural Code and Maritime Fishing.

                  The information which is requested during the constitution of the file is the subject, except justified opposition on behalf of the Customer, of a computer recording. Only staff members of the structure have access to this file: veterinarians and non-veterinarian employees.

                  All veterinarians are required to respect professional secrecy under the conditions established by law (article R242-33 of the Rural and Maritime Fisheries Code). All non-veterinarian employees are also bound by professional secrecy (article 17 of the National collective agreement for veterinary practices and clinics of July 5, 1995. Extended by decree of January 16, 1996 JORF January 24, 1996).

                  The Customer has a right of access, rectification, opposition, deletion, portability and limitation of the processing of information concerning you, by making the request to the file manager, Dr. . Magali MARY*.

                  *EU Regulation 2016/679 of 27 April 2016 on the protection of individuals with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Regulation on data protection)

                   

                   

                     13. Disputes
